Bloomington Connects for Health Grants 2012

General information
Bloomington Public Health will award between $2,500 and $10,000 to assist others working with low-income Bloomington residents. Grant awards must be used to connect residents to the people, processes and systems they need to live healthy, productive and meaningful lives. Funds are provided by Blue Cross Blue Shield.

Eligibility
Any organization may apply for a Bloomington Connects for Health grant. Successful proposals will meet the following criteria:
- Targets low-income, Bloomington community members
- Demonstrates likelihood of sustainability without subsequent funding
Extra consideration given to proposals with the following:
- A strategy that builds organizational capacities when working with low income community members
- Promotes the involvement of lower income members in policy-, systems- and environmental-related health initiatives such as SHIP initiatives to prevent obesity and tobacco use
Examples of allowable programs/activities include the following:
- Purchase of evidence-based multi-lingual health curricula, instructional supplies, curricula enhancements (videos, books, guides)
- Professional development, workshops, and training for organization personnel
- Development of specific health education training targeting low income population
- Translation of vital community resources
- Resource fairs or targeted communications of available resources
- Cross-cultural, health-related community events
- Low-income population involvement initiatives (presenter fees for education forums, expenses for book clubs, resources for parents)
